PROFORMA FOR MEDICAL CERTIFICATE TO BE OBTAINED FROM AN EYE SPECIALIST BY CANDIDATES APPLYING FOR THE POST OF ASSISTANT LOCO PILOT AGAINST CEN No. 01/2025
I have examined Shri/Smt/Kum……………………………………who has applied for the post of Assistant Loco Pilot in Indian Railways. His/her vision has been tested with reference to the required standard for appointment on the Railways and the results are as below:

It is also certified that he/she has not undergone LASIK Surgery or any surgery to correct refractive error.

This is a medical certificate proforma to be completed by an eye specialist for candidates applying for the post of Assistant Loco Pilot in Indian Railways. It includes:
- Candidate Identification: Name and photograph (same as used in online application), attested by the eye specialist.
- Vision Tests:
- Distant Vision: 6/6 in both eyes without glasses; fogging test must not accept +2D.
- Near Vision: Snellen 0.6, 0.6 without glasses.
- Colour Vision: Must be normal.
- Binocular, Field of Vision & Night Vision: Must be normal.
- Certification:
- Confirmation if the candidate meets or does not meet the required vision standards (to be struck out accordingly).
- Certification that no LASIK or other refractive surgery has been undergone by the candidate.
- Details of the Eye Specialist: Name, registration number, signature, seal, date, and place.
- Candidate’s Signature also required.
This certificate ensures the candidate is medically fit for the critical visual demands of the Assistant Loco Pilot role.
